What a same-day appointment for toothache can do

  • The cause. An examination and, usually, an X-ray show whether the problem is tooth decay, an inflamed nerve, a tooth abscess, a crack or a lost filling.
  • Relief. Many problems can be treated at the first visit, for example with a filling. Root canal treatment or a crown (cap) usually needs more visits.
  • A clear plan. You will know whether you need more treatment and how many visits will follow.

Do you need a same-day dentist appointment?

Call us if you have:

  • toothache lasting more than 2 days or not eased by painkillers
  • severe, throbbing or constant toothache, or pain that wakes you at night
  • pain when biting, red gums, a bad taste in your mouth or a high temperature
  • swelling in your gum, cheek or jaw
  • a broken or cracked tooth, or a filling, crown, bridge, veneer or denture that has come loose or fallen out
  • bleeding or severe pain after a recent tooth extraction

A small chip that does not hurt can usually wait for a routine appointment.

Knocked-out adult tooth: time matters

A knocked-out adult (permanent) tooth needs treatment, ideally within an hour. Call straight away, even if you cannot find the tooth. First aid:

  • Hold the tooth by the crown (the white part), not the root.
  • If it is dirty, rinse it briefly in milk, saline or saliva. Do not scrub it or let it dry out.
  • Try to push it back into its socket, then bite gently on a clean cloth.
  • If it will not go back in, keep it in milk or saliva and see a dentist straight away.
  • Do not put a baby tooth back in: this can damage the adult tooth underneath.

When to go straight to hospital

Call an ambulance on 194 or the European emergency number 112, or go to hospital straight away if you have:

  • difficulty breathing, speaking or swallowing
  • swelling around your eye or neck, or changes in your vision
  • a lot of swelling in your mouth, or difficulty opening it
  • swelling with a high temperature and severe pain
  • a serious face or jaw injury, or bleeding from the mouth that will not stop
  • a blow to the head followed by loss of consciousness, vomiting or double vision

Do not drive yourself, and take a list of your medicines.

Your same-day appointment, step by step

  1. Call us. Tell us where it hurts, for how long, and whether you have swelling or a temperature. During opening hours we try to see you the same day if a slot is available.
  2. Your medical history. Tell us if you are pregnant, take any medicines (such as blood thinners or bone-strengthening medicines), or have a heart condition, a weakened immune system or allergies, as these affect the plan.
  3. Examination and X-ray. An X-ray is usually needed, although cracks often do not show up on it.
  4. Explanation and options. We explain the cause, your options (including extraction or waiting), how many visits would follow and what affects the cost.
  5. Pain relief. Under local anaesthetic we may, for example, drain an abscess, start root canal treatment, place a filling or reattach a broken piece. A tooth that cannot be saved is taken out.
  6. Advice and next visit. Aftercare advice and, if needed, an appointment to complete the treatment.

Root canal treatment usually takes 1–2 appointments, and a crown needs a few more visits. A knocked-out tooth that has been put back is checked after about 2 weeks.

Will it hurt? Anaesthesia and pain

Before treatment, we numb the area with local anaesthetic. The aim is for you to feel no pain during the procedure.

A badly inflamed tooth, especially a lower back tooth (molar), can sometimes be harder to numb. A numb lip does not always mean the tooth is fully numb. If you can still feel something, tell us: we can give you more anaesthetic.

If you feel anxious about dental treatment, please tell us. We will explain each step before we do it.

Before your appointment and after treatment

While you wait to be seen:

  • Take ibuprofen or paracetamol exactly as the leaflet says. A higher dose will not work better and can be very dangerous.
  • If you have a long-term condition, take other medicines or are pregnant, ask a pharmacist which painkiller is right for you. See also Dental care in pregnancy.
  • Do not put aspirin on the tooth or gum, or give it to anyone under 16.
  • Rinse with warm salt water, then spit it out (not for children).
  • Eat soft food and avoid anything sugary, very hot or very cold. Do not smoke.
  • Do not ignore facial swelling: an infection can spread quickly.

More advice: Toothache: what to do until you see a dentist.

After treatment

  • The tooth may be sore for a few days.
  • Do not eat until the numbness has worn off (usually 2–4 hours), so you do not bite your cheek or tongue.
  • Take any prescribed medicines exactly as directed.
  • Keep your follow-up appointment: a temporary filling is only a short-term fix.
  • Call us if swelling gets worse, you develop a temperature, bleeding will not stop or painkillers do not control the pain.

Risks and alternatives

Not every tooth can be saved. A split tooth, or one with a crack running below the gum line, usually cannot be kept. A vertical crack in the root may mean the tooth has to come out.

Cracks in teeth do not heal. A crown protects a cracked tooth, although the crack can sometimes still spread. Even so, most of these teeth keep working comfortably for years.

An untreated infection can spread quickly and become dangerous.

Antibiotics do not cure ordinary toothache: they do little or nothing for the pain, have side effects and contribute to antibiotic resistance.

The decision is yours. Read more about when root canal treatment is better than extraction.

How much does toothache treatment cost?

The cost depends on what the examination shows. The main factors are:

  • whether you need an X-ray
  • the cause and the treatment: a filling, root canal treatment (number of canals and visits), draining an abscess or an extraction
  • whether a temporary repair is done now and the final one later
  • whether a lost tooth needs replacing with a bridge, implant or denture

You will know the cost after the examination, before treatment starts. For longer treatments, you will receive a written plan. See our prices page.

The practice has a contract with the Health Insurance Fund (FZO) for primary dental care. If Dr Kiril Milanovski is your chosen dentist (izbran stomatolog), please mention this when you call.

Toothache and broken teeth: frequently asked questions

What should I do about severe toothache?

Take ibuprofen or paracetamol as directed and chew on the other side. Call us if the pain lasts more than 2 days, does not ease, or comes with swelling or a temperature.

Will painkillers or antibiotics help my toothache?

Ibuprofen or paracetamol can ease the pain, but they do not treat the cause. Antibiotics are mainly needed when an infection is spreading and you have a temperature or feel unwell, or if your immune system is severely weakened. Your dentist or doctor will decide.

Where can I find an emergency dentist in Skopje?

We are not an emergency or out-of-hours service. Monday to Friday, 11:00–19:00, call us on +389 78 218 457 and we will try to see you the same day. Outside these hours, contact the out-of-hours dental service run by public health institutions.

What should I do if I chip or break a tooth?

Rinse your mouth with warm water and, if your face is swollen, hold a cold compress against it. Keep any broken piece in milk or saliva and bring it with you: it can sometimes be reattached. If the tooth feels sharp or sensitive, bite on clean, damp gauze.

Is a swollen face from toothache an emergency?

A swollen gum or cheek should be checked the same day, as it can mean a spreading infection. If the swelling spreads towards your eye or neck, or you have a high temperature or difficulty breathing or swallowing, call an ambulance on 194 or the European emergency number 112.

Why do I have toothache and earache together?

It can be a sign of a dental abscess, but also of an ear or throat infection. Have your teeth checked first and, if they are fine, see your doctor.

The pain has stopped. Do I still need to come in?

Yes. Pain can come and go while the problem remains. A dull ache that lingers may mean the nerve is damaged or dying, and an abscess does not clear up without treatment.

If you are in pain now, call us on +389 78 218 457 during opening hours. For a problem that can wait a few days, request an appointment using our booking form.
